Homer Couch is a board-certified Psychiatric Mental Health Nurse Practitioner dedicated to providing compassionate, patient-centered mental health care across the lifespan. With a strong clinical foundation and a passion for helping individuals achieve emotional wellness, Homer specializes in the assessment, diagnosis, and treatment of a wide range of mental health conditions, including anxiety, depression, mood disorders, trauma-related disorders, and ADHD.

Committed to creating a supportive and nonjudgmental environment, Homer believes in partnering with patients to develop individualized treatment plans that may include medication management, supportive therapy, and lifestyle interventions. He prioritizes open communication, education, and empowering patients to take an active role in their mental health journey. At Rise Above Mental and Behavioral Health, Homer currently provides telehealth services — increasing access to care and meeting patients where they are. In-person services are planned for the future.

Tiffanie D. Conley is a Kentucky Licensed Clinical Social Worker with more than 20 years of experience in behavioral health and human services. She earned her Bachelor of Social Work from Eastern Kentucky University and her Master of Social Work from the University of Kentucky.

Throughout her career, Tiffanie has worked across psychiatric, addiction recovery, community-based, outpatient, crisis, and behavioral health settings. Her clinical experience includes trauma-informed care, substance use disorders, co-occurring mental health conditions, crisis intervention, risk assessment, involuntary evaluations, treatment coordination, and multidisciplinary collaboration.

Tiffanie's professional background also includes extensive experience evaluating behavioral health needs, determining appropriate levels of care, advocating for individuals and families, coordinating services, and supporting continuity of care. Her work in utilization management and addiction recovery has provided her with a strong understanding of the behavioral health system and the barriers many individuals encounter when seeking treatment.

Tiffanie believes effective therapy begins with a genuine therapeutic relationship built on trust, safety, respect, and understanding. She uses a person-centered, strengths-based, and trauma-informed approach while recognizing that there is no single treatment approach that works for everyone.
